Dear Parents,

It is quite likely that my staff recently asked you to sign a HEALTHIX  CONSENT FORM.
Although we do have an informational poster in our office waiting room, and although my staff, and my self do our best to answer your questions, many of you may still have some questions about what you signed up for , or perhaps did not sign up for.

Healthix is a Regional Health Information Organization, or "RHIO."
Healthix is in fact the largest RHIO in the United States, serving New York City and Long Island
.



WHO IS HEALTHIX?

Healthix is a non-profit, health information exchange funded by the NYS Department of Health. Healthix is committed to securely exchanging patient data to improve the quality and efficiency of care. Healthix provides patient information to thousands of doctors and providers in the Greater New York Area.

Healthix securely gathers your medical information from hospitals, physician offices, urgent care centers, laboratories, radiology centers, and many other organizations and matches that information to create a master patient record.

 As a key partner in the Statewide Health Information Network for NY (SHIN-NY), Healthix  connects hundreds   of organizations, with nearly 1,500 facilities across New York City and Long Island. In other words information is shared not only by Healthix with its member physicians and facilities, but also through SHIN-NY Healthix provides and obtains information with the RHIO's in NY.

Only health care providers caring for you can access this information,
and only with your consent. I, myself can only access your protected medical information (PHI)  from  " external documents," through Healthix with your consent. You have the ability to allow or deny this consent and are therefore requested to sign the provided consent form, to allow me to collect your information to provide the highest possible level of care.
Other health providers called " break the glass providers" are physicians working in emergency rooms who may need your information in life and death situations. I am not a " break the glass" provider. When you enter an emergency facility you may choose to only allow break the glass access in emergency situations .

A recent caveat was that  until recently NY state law had not determined who was allowed to give consent to release PHI  for persons aged 10 through 17 years of age. It was unclear if the parent or the child or some combination was the responsible party.  As of 2018 this situation has been resolved, and thus Healthix is available to patients in our practice of all ages.
